      Hello Dr Woeller,

      I have a new patient that has been given the bile acid sequesterant Welchol for her urgent diarrhea. I understand that when bile is not reabsorbed by the terminal ilium, it stimulates water secretion and peristalsis resulting in diarrhea.

      What I’m not clear on is how to support this problem from a functional medicine perspective.

      Can this type of diarrhea be caused by any of the pathogens we have been studying? The only thing that I can come up with is to investigate causes of inflammation in the terminal ilium and remove those trigger – I haven’t come up with a good list of possible triggers except for gluten.

        Holly,
        Start with diagnostic testing, i.e. OAT, #401-H, Food IgG. If these prove to be clear then consider a bowel and liver detox as outline by Dr. Tranchitella in Module #9. It could be that this individual is harboring bowel toxins that are affecting the intestines leading to diarrhea, if there are not any pathogens present.
